Lutetium PSMA Therapy (¹⁷⁷Lu-PSMA Therapy) is a targeted radioligand treatment used primarily for selected patients with advanced prostate cancer whose cancer cells express Prostate-Specific Membrane Antigen (PSMA). The therapy uses a PSMA-targeting molecule attached to the radioactive isotope Lutetium-177. After entering the body, the molecule binds to PSMA-expressing cancer cells and delivers radiation directly to them, helping destroy or control the cancer while limiting exposure to many surrounding tissues.
Patients with Advanced Prostate Cancer
Individuals with metastatic or advanced prostate cancer that has progressed despite standard treatments and whose tumors demonstrate PSMA expression.
Patients with PSMA-Positive Cancer
People whose prostate cancer has been confirmed to express sufficient PSMA through appropriate imaging, commonly a PSMA PET scan, making them potential candidates for targeted radioligand therapy.
Patients with Previously Treated Disease
Individuals whose cancer has progressed after appropriate hormone-based or other systemic treatments and who meet clinical criteria for Lutetium-177 PSMA therapy.
PSMA PET Evaluation
Your oncology and nuclear medicine team performs PSMA-based imaging and other medical assessments to confirm that the cancer shows sufficient PSMA expression and that the therapy is appropriate.
Targeted Lutetium-177 Infusion
Lutetium-177 PSMA is administered intravenously. The PSMA-targeting molecule attaches to cancer cells, delivering beta radiation directly to PSMA-expressing tumor cells.
Response Monitoring & Follow-Up
Your medical team monitors blood counts, kidney function, treatment response, and potential side effects through laboratory tests, imaging, and clinical assessments throughout the treatment course.

Before Procedure

- Blood tests, kidney-function evaluation, and other medical assessments are typically performed before treatment.
- Your specialist will explain the treatment schedule, radiation-safety precautions, potential side effects, and supportive care requirements.
During Procedure
- Lutetium-177 PSMA therapy is generally administered through an intravenous infusion under the supervision of a nuclear medicine team.
- The treatment itself may take several hours, including preparation, infusion, monitoring, and hydration.
- You may receive intravenous fluids and other supportive measures to help protect kidney function and manage treatment-related effects.
After Procedure
- You will receive specific radiation-safety instructions, which may include temporary precautions regarding close contact with others, particularly children and pregnant individuals.
- Fatigue, dry mouth, nausea, reduced blood-cell counts, or changes in kidney function may occur and are monitored by your medical team.
- Treatment is commonly delivered in multiple cycles several weeks apart, with blood tests and imaging used to assess response and determine whether further cycles are appropriate.
